It is the patient's responsibility to provide us with your insurance information at your initial office visit, and to update us with any changes.

We accept most insurance plans. Please ask if yours is accepted.

Many insurance plans require the patient to pay a Co-pay at the time of an office visit and/or to present a referral. These are mandated by the insurance company, and failure to collect the Co-pay or receive the referral is a breach of our contract with the company. We therefore cannot see a patient without payment of the necessary Co-pay and presentation of necessary referrals.

While we will be happy to assist you with questions of insurance coverage, it is ultimately your responsibility to ensure that your insurance plan covers the evaluations and treatments recommended. Otherwise, you will be responsible for the fees.
